Emceed by 10TV's Angela An, this annual meeting is an opportunity to reflect on 2023, get insight into the year ahead, and formally pass the gavel to the new Chairman of the Board, Mark Smolik (DHL Supply Chain).
Attendees will also experience an Innovation showcase where new Westerville business, Form5 Prosthetics will demonstrate their latest technology: affordable, custom-application prosthesis devices.

Dr. Brandon Kozar, Psy.D., MBA
Director of Leadership Coaching & Development, Director of YOU Matter Program
Nationwide Children's Hospital
Dr. Kozar is a licensed clinical psychologist, Director of Leadership Coaching & Development, and Director of the YOU Matter staff support program for Nationwide Children's Hospital. He is the founder of Kozar & Associates, LLC, a management consulting, coaching, and succession planning firm. His primary responsibilities involve overseeing all mental and emotional health services for NCH hospital staff; and developing hospital leadership, coaching chiefs of medicine in growing and managing their divisions. Dr. Kozar specializes in leadership coaching, interpersonal and team dynamics, personality assessment, and employee mental health. He is a frequent presenter on topics related to leadership, emotional intelligence, stress management/resiliency, employee mental health, and interpersonal communication and conflict negotiation.
Born and raised in northeast Ohio, Dr. Kozar graduated Magna Cum Laude from John Carroll University with a Bachelor's of Science in psychology. He concurrently achieved his Doctorate in Clinical Psychology and his Master's in Business Administration with a specialty in organizational development and leadership from Wright State University.

Form5 Prosthetics
Aaron Westbrook emerged as a pioneer in prosthetics in 2015. Born with only one hand, his journey into prosthetics began as a freshman at New Albany High School, after personally encountering limitations and gaps in upper limb prostheses. Undeterred, at age 14, Aaron embarked on a do-it-yourself mission to create his own using a 3D printer.
Aaron's determination extended beyond personal triumph. Having connected with the limb difference community through his "Alive With Five" blog, he aspired to make this technology accessible to others. Inspired by his vision, a successful Kickstarter campaign enabled him to purchase a 3D printer, leading to the establishment of Form5 Prosthetics as a 501(c)3 non-profit in 2017. While growing Form5, Aaron completed rigorous coursework at The Ohio State University Fisher College of Business and received his undergraduate degree in 2022.
As a CEO, Aaron has propelled Form5 along a remarkable trajectory, guided by its strategic pillars of Community, Research & Development, Education, and Sustainability. The organization consistently realized its vision -- a world where individuals with limb differences possess the confidence, resources, and support to achieve their dreams.
Aaron Westbrook's transformative journey exemplifies the power of one person's determination to make a positive impact, and Form5 stands as a beacon of hope and empowerment for those with limb differences.
