About Us

Form5 Prosthetics began with Founder & CEO Aaron Westbrook. Born with only one hand, Aaron did not receive a prosthesis until high school (2014). Unfortunately, it did not fit well, was expensive, and he would outgrow it quickly. Disappointed but not defeated, Aaron embarked on a mission to make his own prosthesis. What started as a journey to raise awareness led him down a path of innovation in developing 3-D printed devices for himself and others with limb differences.

Founded in 2017, Form5 empowers people to interact successfully with their world and future. Form5 generates impact through its strategic pillars: Community, Research & Development, Education, and Sustainability.

Form5 cultivates a community of individuals with limb differences through its Nub Club of Ohio.

Form5 builds interdisciplinary teams of specialists that research & develop custom-application devices. Form5's Bike Arm Adapter is patent-pending.

Form5's educational programs and STEM curriculum spark social innovation, creating novel Form5 devices.

Form5 is committed to organizational sustainability through its social enterprise, Difference Makers 3D (DM3D), providing design and 3D printing services.
